本项目为上海市浦东医院科教楼外立面维修工程：本建筑建造于2019年，地上10层，地下二层，建筑面积(略).2平方米，占地面积3377.4平方米，耐火等级一级，建筑高度55米，外立面面积约9732平方米。 建筑现状：原外立面真石漆及保温材料出现多次脱落，存在重大安全隐患。 设计要求：外墙原有保温及真石漆材料整体拆除，翻新改为外墙保温一体铝板； 项目概算：约1270.87万元,其中建安费1133.55万元，二类费137.32万元（投标请以此概算额报价，最后结算支付以审计价为准）。 服务时限：一年。

(1)设计费报价应包括完成本采购项目设计所有工作量和提供全套设计文件及相关配合工作及服务的全部费用，主要包括：方案设计、施工图设计、提供设计技术交底、解决施工中的设计技术问题、参加竣工验收以及施工现场后续服务等费用。 (2)本次设计费报价实行市场调节价，各供应商自主报价。由于非供应商原因造成建设项目设计工作量增加或减少的，采购人应当按合同约定与成交供应商协商另行支付或扣减相应的设计费用。 (3)供应商报价应包括国家规定的增值税税金，增值税税金按一般计税方法计算。 (4)设计报价依据：项目概算及沪浦发改投（2012）339号。 (5)设计报价最高限价为39.49万元。

（四）在中介集市报名后中介服务合同签订前，存在记入一般或者严重不良诚信档案行为的
